Permission Slip 

Permission Slips need to be turned in at least 7 days prior to the event.  Unless otherwise stated on the Schedule of Events page. 

Website Permission

We would like to post pictures that were taken during Youth Ministry events.  Please have a Parent or Guardian sign the form so your picture can be taken.
